Our Mission
Community Visiting Nurse Association and our affiliates, Community Visiting Nurse Service and Community Care Hospice are community based, not-for-profit health care agencies that provide and coordinate quality home care, hospice and community services and promote health education, wellness, and prevention services.
Our programs at CVNA include:

Community Visiting Nurse helps patients regain and maintain independence while they are recovering from an injury, surgery or hospitalization, and can ease the management of an ongoing medical condition or chronic illness such as diabetes, heart failure or COPD, in the comfort and security of their own home.

Its about Dignity, Comfort and Peace. Hospice care focuses on the patient's comfort and quality of life with a team of compassionate professionals who are here to help during the end of life journey. We work together, as a team, to improve quality of life, minimize discomfort, and control symptoms.

Keeping our communities healthy is an integral part of Community VNA’s mission. Community Home Care offers programs and services targeted to Somerset and Middlesex County residents in their homes and at community sites, providing assistance and education for all age groups, from newborns to seniors.
