Our Services Include
Skilled Nursing
- Direct care such as wound care, ostomy care, etc.
- Observation and evaluation of patient needs
- Communication of progress with primary physician
- Coordination of care with other disciplines
- Patient/caregiver education on disease processes, self-care techniques, medication management, prevention strategies and others
Rehabilitation - Physical, Occupational and Speech Therapy provide;
- Therapeutic exercise to improve strength and balance
- Transfer and/or gait training
- Recommendations for equipment or assistive devices
- Instruction in activities of daily living (bathing, dressing)
- Address speech language deficits or problems with swallowing
- Home Safety and patient/caregiver education
Medical Social Services
- Short term counseling services
- Coordination with community resources
- Assistance with long range planning
Nutritional Counseling
- Registered dietitian provides guidance and education on therapeutic or modified diets
- Providing support for chronic diseases and guidance on enteral and parenteral nutrition
- Supporting care plans and enhancing comfort through guidance on foods and liquids
Certified Home Health Aides
- Provided under the supervision of the registered nurse
- Assistance with activities of daily living (bathing, toileting, dressing, etc.)
- Provides support to patient and family
