Hospice is a specialized, interdisciplinary approach to caring for individuals with a life limiting illness. Our team has the experience, compassion and resources to guide families through this very personal time in their lives. The focus is on the emotional, spiritual and physical needs of the patient and family through understanding, support, symptom management and comfort measures. For us it is truly about Dignity, Comfort and Peace.
Our team is coordinated by our Medical Director and Hospice Manager and supported by our Nurses, Social Worker, Chaplain, Pharmacist, Bereavement Coordinator, Volunteers and Hospice Aides.
Care can be provided:
- In the home
- Nursing home
- Assisted Living Facility
- Other living situations
Hospice may be appropriate when:
- Symptoms are becoming harder to manage
- Spending more time in the hospital with less time between admissions
- Priority is comfort and relief of pain or other symptoms
- Illness is in a life limiting stage
