How do I obtain services from Community VNA?
Obtaining Community VNA’s home health care services begins with a call from you, your family or your physician to our Intake Department at 908-895-2222. We will contact your physician first, then call you to schedule an appointment to assess the care you need.

How is Home Health Care Paid for?
Medicare Part A and/or Medicare Part B, Medicaid, and Managed Medicare cover eligible home health care services—as long as you need part-time or intermittent skilled services and are considered home-bound or have difficulty leaving the home. Our staff will determine eligibility on your behalf prior to initiating services.
Managed Care and other Private Insurance generally cover skilled home health care services, though there may be copays required. It depends on the insurer and the plan you have. Our intake coordinator will determine eligibility and if there are any copays prior to you accepting services.

Community Care Hospice provides services to all of Somerset County, Middlesex County, Hunterdon and Warren Counties.

What criteria do I need to meet to be eligible for Hospice care under Medicare?
You qualify for hospice care if you have Medicare Part A (Hospital Insurance) and meet all of these conditions:
-Your hospice doctor and your regular doctor (if you have one) certify that you’re terminally ill (with a life expectancy of 6 months or less).
-You accept comfort care (palliative care) instead of care to cure your illness.
-You sign a statement choosing hospice care instead of other Medicare-covered treatments for your terminal illness and related conditions. -
What criteria do I need to meet to be eligible for Hospice care under Managed Care or Private Insurance?
Most private health plans align with Medicare in their requirements for hospice: A patient must be diagnosed with a terminal illness (indicating a life expectancy of six months or less) and must choose not to receive curative treatment.
Individuals who have coverage from private insurance should contact their health plan directly for specific details on hospice care, including what the patient’s plan will cover and which out-of-pocket costs the patient and their family may be responsible for. Our intake department can assist with this, with your permission.
