Strategic Healthcare Programs (SHP) is a leading provider of performance improvement software for post-acute providers, including hospice, home health, hospitals and skilled nursing facilities.  They provide real-time analytics and benchmarking for performance measurement.  Today they serve over 7,600 healthcare providers. 

Community Care Hospice achieved a top 5% overall score for caregiver experience for 2024 as measured against all SHP Hospice clients.  Community Care Hospice has served the communities of Somerset and Middlesex Counties since 1984 and just recently added Hospice services in Warren County.  Their dedication, genuine caring, expertise and support has been recognized by both patients and families.  Hats off to our staff for this incredible achievement!
