Community VNA has an impressive history of providing high quality home health care to Somerset and Middlesex County residents since 1948, and to Warren County residents since 2014. As on organization, we focus on the development of staff and programs to meet the health needs of local residents. The quality services we provide, our leadership in our field and our commitment and contribution to individuals and the community is why our caring staff makes a difference.
Our original charter in 1948 pledged to:
- Promote individual, family and community health;
- Prevent disease by teaching the principles of health, hygiene and sanitation (infection control);
- Apply the functions of public health nursing in the prevention of disease and promotion of health.
Our purpose today—over 75 years later—remains largely unchanged. We’ve enhanced our services to include a multidisciplinary team of clinicians, working together to promote health and prevent disease. Our mutual goals are to improve or maintain health, manage disease symptoms, restore function, promote safety and prevent rehospitalizations.
Our Mission
Community Visiting Nurse Association (Community VNA) and our affiliates, Community Visiting Nurse Service and Community Care Hospice, are community-based, non-profit health care agencies that provide and coordinate quality home health services, hospice care and community services and promote health education, wellness and prevention to our communities.
