Community VNA’s service begins with a call from you, your family or your physician to our Intake Department. If the call is from you or your family, we’ll contact your physician and set up a visit with you to determine how best we can help. If the call comes from your physician, we will contact you to schedule a visit with one of our staff to assess the care you need.
From the beginning, we involve you in your plan of care, and, when appropriate, teach you, your family members or caregivers about your treatments or the therapies ordered. Regular visits by our staff will provide the care you need. Our goal is to help you manage your illness, improve your health and maintain your independence in your home or residence.
Please call our Intake Department (908-725-9355, Ext. 2222) Monday through Friday, 8:30 a.m. - 4:30 p.m. with your questions or requests. If you prefer, you can use our easy fax referral form. Just print it out, complete it and fax it to us at 908-722-3014. If a call is received or a fax is sent over the weekend, our Intake Department will call to confirm receipt and acceptance on the next business day.
For non-urgent, after hour referrals, please fax the referral form to our Intake Department. Your referral will be received and processed on the next regular business day.

New Medicare Requirement: "Face to Face"
Medicare has issued a new “Face to Face Encounter” requirement for individuals seeking home health care. Effective April 1, 2011, physicians must certify a patient’s need for home health care through a face to face/in-person visit within 90 days prior to or 30 days after the start of home health services. Community VNA will be working with community physicians, hospitals and other health care facilities to implement this new requirement.
