Community Visiting Nurse Association is a non-profit, fully accredited Home Health Care agency serving Somerset, Middlesex and Warren Counties. We are a 4 Star Medicare certified agency providing comprehensive and quality home care services to individuals within their homes.  

We currently have job opportunities available based out of both our Somerville and Phillipsburg offices.  

Somerville Office

Receptionist/Office Assistant - Full-Time. Answers, screens and forwards incoming telephone calls. Greets all visitors. Processes incoming mail. Performs administrative tasks. Requirements include: proficient computer skills including Microsoft Office, excellent communication and organizational skills.  

Speech Language Pathologist - Per Diem SLP position for Middlesex County weekdays to provide skilled therapeutic services to individuals with communication and swallowing disorders and other varied health care needs in their homes. 

Hospice Nursing - Per Diem RN position for Middlesex/Somerset Counties weekdays with one weekend per month/two holidays per year requirement; back-up to Hospice staff as needed a plus. Two years nursing experience with hospice, home care/or med-surg preferred.

Skilled Nursing - Per Diem RN positions. One weekend per month and two holidays per year requirement. Two year nursing experience with home care and/or med-surg experience preferred.

Home Health Aide - Must have reliable transportation and be certified as a Home Health Aide in the State of New Jersey. One year of home health aide experience required or be a recent graduate of an accredited Certified Home Health Aide program.   

Phillipsburg Office

Skilled Nursing – Full Time and Per Diem RN positions for Warren County, 8:30 AM – 4:30 PM Monday- Friday.  One weekend per month and two holidays per year requirement.  Two years nursing experience with home care and/or med-surg experience preferred. 

Skilled Nursing – Per Diem Weekend On-Call RN position for Warren County, to work Friday and Saturday, 4:30 PM – 8:30 AM.  Two years nursing experience with hospice, home care and/or med-surg preferred.  

Physical Therapist – Per Diem Physical Therapist position for Warren County weekdays to provide skilled, therapeutic rehabilitation services to individuals for whom such therapy has been medically prescribed. 

RN Discharge Planner/Liaison – for Warren County to coordinate patient care between hospitals, skilled nursing facilities, physician’s offices and our agency.  This position will assist in facilitating the discharge planning process in order to help transition the patient to home health care services and promote continuity of care.   

Nursing qualifications include:

·         Excellent written and verbal communication skills

·         Critical thinking, able to function independently

·         Proficiency with computers and electronic medical records

·         Ability to case manage, organize schedule and manage time effectively

·         Ability to assess individual client needs, provide direct care and patient teaching

·         Development of a comprehensive plan of care

·         Knowledge of community resources

·         Graduate of an approved college or school of nursing

·         Current New Jersey Registered Professional Nurse license

·         Two years of home health and/or med/surg experience preferred

·         Ability to work one weekend per month and two holidays per year are required
